The role
A well-respected Local Authority in the Caerphilly area is seeking an experienced Qualified Social Worker to join their Community Team. This is an excellent opportunity for a skilled practitioner looking to take on a varied caseload within a supportive and established service.
You will play a key role in completing assessments and managing ongoing care arrangements to support adults within the community to live safely and independently.
Key Responsibilities:
Undertake strengths-based social care assessments
Manage a diverse community-based caseload
Arrange and review Packages of Care
Coordinate Direct Payments
Support placements in Extra Care Housing, Supported Living, and Residential/Nursing homes
Work collaboratively with multi-disciplinary professionals and partner agencies

About You:
You will be a qualified and registered Social Worker with experience in adult services and community-based practice. Strong assessment, care planning, and case management skills are essential, along with a commitment to delivering person-centred outcomes.
