Job overview

Croydon Outcome Based Commissioning Alliance has developed six Integrated Care Networks (ICNs) across the borough. A central feature of the ICNs is the advent of ‘GP huddles’ which ensure that patients regarded as medium to high-risk are identified quickly
and proactively case managed as a result of this. The huddles occur on a weekly basis and the intention is to allocate Community Psychiatric Nurses to each ICN forming a Primary Care Liaison team with specific responsibilities for attending the huddles.

The team will be based at Heavers Resource Centre but will primarily work within GP surgeries. The post-holder will be expected to line manage team members and provide clinical supervision in addition to attending huddles within their allocated area. Part of the role is to offer advice signposting for patients, in collaboration with other members of the MDT, with patients that have mental health issues that can be treated in primary care.

To act as Team Manager - providing senior nurse leadership to team members. To have overall management responsibility for all aspects of the Primary Care Liaison Service (including operational, clinical, information and financial management).

To ensure that the services provided by the Primary Care Liaison service are high quality, well organised, responsive, needs-led, and user and carer focussed.

Record the activity on ePJS and EMIS which will identify the need for mental health support across the huddles and help scope the work and produce regular updates and a yearly report.

To attend weekly GP huddles and offer comprehensive initial assessments and short term case management for patients in collaboration with other members of the MDT.

To work with the MDT to ensure that the care offered is evidence based.

To signpost patients under the age of 65 following assessment to relevant SLaM services.

To effectively assess and manage complex psychiatric and physical health risk as required. To maintain accurate clinical documentation and complete outcome measures as per Trust policy.

Essential criteria

·Registered Nurse-Mental Health (A)

·Mentorship Course (or other relevant teaching/assessment course) (A)

·Evidence of continuing professional development (A/I)

Desirable criteria

·Master level qualification (A)

·Further training relevant to Older Adult Mental Health care (A)

Experience

Essential criteria

·Experience of assessment and casework which meets the needs of older adults with mental health problems who are supported in the community (A/I)

·Experience of assessing and managing risk in the context of community work with people who have mental health problems (A/I)

·Experience of providing clinical and managerial supervision (A/I)

·Experience of working within a multidisciplinary team (A/I)

Desirable criteria

·Experience of implementing significant changes in working practices

·Experience of working with other client groups and of using a variety of models/approaches to care (A)

Knowledge and Skills

Essential criteria

·Specialist clinical knowledge relevant to the service, including evidence-based practice.

·Knowledge of current issues in the care of people with acute mental health problems within the community (I)
