Main duties of the job
The successful candidate will beassessing and making appropriate and timely discharge plans for patientsunder the supervision of a registered nurse. Good communication skills, a pro-active approach towards discharge planning and the ability to work flexibly to meet the needs of the service is essential. They shouldbe able to problem solve complex situations and recommend appropriate action.

There is a planned move for this service during 2025, when this role and base of work will relocate to Bournemouthsite. Upon relocation of the role/service, excess mileage allowance / reimbursement will not apply, and any travel between the post holder's home and work base will be classified as a commute. Any other changes unrelated to this, will be handled in line with Trust/National terms and conditions.

1. Experience of working in a hospital setting or relevant community service
2. Understanding of complex discharge planning in care of the elderly, stroke and vulnerable adults.
3. Experience of supporting patients' specialised care needs to enable informed decision making

4. Previous experience of working with patients requiring CHC and Social care involvement
Technical Skills Competencies

5. Assessment, planning and organisational skills
6. Ability to progress plans for discharge with internal / external agencies and family members
7. Use of IT including Windows, Word, Excel, Powerpoint,
