About Cornwall Supported Living: Our specialist Supported Living and Outreach service Cornwall, provides support for people with Learning Disabilities, Autism, Mental Health needs and Acquired Brain Injuries. We understand the importance of providing person-centred support that enables independence and meets each individual's needs.
Responsibilities
* Managing a team of Support Workers
* Providing high standards of care and support for people we support, in line with care and support plans
* Encourage good community relationships by promoting a variety of activities
* Accurately report, record and maintain records as required and working to care plans
* Liaise with health professionals and their families
* Complete supervisions and audits
* To ensure that all duties undertaken by reporting staff are performed to required standards, and that all such staff contribute in a positive manner to provide a happy, efficient and homely atmosphere within the Home.
