Responsibilities
* Provide support and guidance to all participants that have been registered onto the project.
* To develop ongoing action plans with participants which will be reviewed on a regular basis.
* To provide one to one support and group activities to aid the development of a participant in their recovery.
* Work alongside the team structure in delivering services and outcomes for the scheme.
* To achieve project targets and outcomes set by Welsh Government.
* To support the team leader and referral agents in raising awareness of the service.
* To work in partnership with other practitioners to deliver effective interventions and support for people with Mental Health and/or substance misuse.
* Provide support and supervision to volunteers.
