We are seeking to appoint a dynamic, enthusiastic and highly motivated forward-thinking Band 7 Clinical Nurse Specialist, who would be adopted by MacMillan, to work within the Specialist Palliative Care team. There are 2 posts: 1 x 37.5 hours and 1 x 22.5 hours. These are predominantly hospital-based positions but form part of a team of specialist palliative care nurses covering the hospital and community seven days a week, including bank holidays, also supporting the community as staffing requires.
This service is part of the Community Division, which provides a multitude of services within Bolton. Experience of working in the community is not essential, as the successful candidate will receive a full supportive induction when they join the Division and the team.

Responsibilities
* Be a proven team player with excellent communication and teaching skills, with significant, relevant experience.
* Contribute to the provision of specialist palliative and end-of-life care to the people of Bolton with advanced life-shortening disease, working autonomously and as part of a team.
* Deliver clinical interventions across care settings, providing advice, education and support to patients, their carers and other health care professionals.
* Promote advance care planning, management of complex end-of-life symptoms and patient and carer expectations while meeting integrated care needs and preventing inappropriate hospital admissions.
* Contribute to the development of services, teaching, audit and research.

Specialist Palliative Care services have been developed in an integrated and collaborative manner between the hospital, community and Bolton Hospice, reflecting excellent cooperation and a patient-centred whole-team approach.
