This innovative team works collaboratively with NHS colleagues and partners from Community First, hospital flow and discharge services, reablement, social work, and care-at-home teams. Our core focus is to: Prevent unnecessary hospital admissions Support timely and safe discharges Reduce the impact of unscheduled care Promote recovery at home As a Social Worker in this team, you will: Adopt an “In Reach” approach within Edinburgh hospitals Carry out assessments for individuals in the community Deliver responsive, person-centred support to enable independence and recovery Working Pattern Currently Social Work element of this service operates Monday to Friday. There is the Potential for future seven-day working, with weekend shifts anticipated up to two weekends per month. While eligible for the Council’s flexible working scheme, the nature of this role demands a high level of agility and responsiveness. As such, set working days cannot be guaranteed, and on-site working will be essential, with limited scope for homeworking.
