GP Practice Nurse - fixed term maternity cover
West Leeds Family Practice is recruiting a fixed term Practice Nurse to cover a period of maternity leave. Our surgery consists of 5 Partners 10 salaried GPs, 2 ANPs, 3 nurses and 5 HCAs. We are part of the West Leeds Primary Care Network and work closely with our neighbouring practices. We are located across 3 sites, are a teaching practice and host both medical and nursing students
We have 20k patients and are continuously expanding
We are a system 1 practice who are high achievers in QOF,DES, LES and all flu and vaccine services.
We are very flexible regarding days and times of work. This would suit an experienced practice nurse who did not want to commit to full time work, but enjoys working as part of a dynamic team.
Main duties of the job
To be fully proficient in Chronic disease management,vaccinations and immunisations, cervical screening, contraceptive reviews,wound care including compression, as well as other general treatment roomduties. It is not expected that the successful applicant has to be proficient in all these areas, however managing long term conditions is a pre requisite.

The nursing team is extremely close working and mutually supportive. We have a nurse educator who works both for the practice and the PCN. The successful applicant will be supported in growing their expertise and encouraged to have their say.
Job responsibilities
Provide Chronic disease management for Diabetes, COPD,Asthma, and CHD/CVD
Perform cervical screening and promote breast awareness
Administer adult and paediatric injections
Provide adult and child immunisations ensuring fullachievement of targets
Ensure safe storage, rotation, and disposal of vaccines anddrugs
Undertake venepuncture and electrocardiography
Undertake wound care/ management / dressing techniques
Provide advice on lifestyle issues, assisting and supportingpatients in changing their unhealthy behaviours and refer appropriately
Ensure that current evidence-based approaches to patientcare are standard practice
Apply infection control measures when collecting andhandling of laboratory specimens, disposing of waste materials, dealing withblood and body fluids spillages and report and treat sharps injuries
Audit outcomes of care against standards and initiate changeas necessary ensuring follow-up audit cycle
Maintain and update disease registers

Qualifications
* Qualified Registered Nurse with a current NMC registration
* Experience of working in a GP practice and managing long term conditions
