At Haven House Children’s Hospice, we are dedicated to providing the very best care for seriously ill and life-limited children, young people, and their families. We believe every moment matters. From birth, through childhood, adolescence and into young adulthood, our care supports families in local communities facing the most difficult challenges.
We are now seeking a compassionate and experienced Social Worker (SWE registered) or Registered Nurse with Designated Safeguarding Lead experience to take on the role of Family Support and Transition Lead. This is an exciting role, bringing together three vital areas: family support, safeguarding, and transition planning. You will be a visible leader and first point of contact for families who need practical, emotional, and social support, ensuring they are listened to, empowered, and able to access the right services at the right time.
Location & Hours
Based at our woodland hospice in Woodford Green, North East London, with free parking and easy access from the Central line.
Hours: 28 hours per week (4 days).
Salary
£42,025 – £49,199 pro rata (Band 5, dependent on experience).
Responsibilities
* Lead the Family Support Service, including social care advice, advocacy, and practical support.
* Act as Deputy Safeguarding Lead, supporting the organisation to keep children and families safe.
* Take the lead on transitions, ensuring young people are supported as they move into adult services.
Qualifications
* Qualified professional (social work or designated nurse equivalent) with experience in family support, safeguarding, and partnership working. SWE registered or equivalent professional registration is required.
