Overview

Job summary:

* Providing medicines advice to patients and carers, health and social care professionals and skilled non-registered staff working collaboratively to resolve complex medication queries.
* Medicines reconciliation on admission and discharge and clinical medication reviews.
* Making evidence-based clinical interventions and recommendations to prescribers to provide patient-centred care.
* Supporting patients in managing their medication safely, including facilitating self-administration.
* Work towards a non-medical prescribing qualification.
* Collaborating with a multidisciplinary team (including medical and ACP prescribers, nurses, physiotherapists, occupational therapists, primary care, community pharmacy, mental health teams, care providers and social care staff professionals).
* Provide a high quality, safe and patient focused clinical pharmacist service to the Community Care Group, including community hospital inpatients and patients in their own homes.

We are looking for an experienced pharmacist to join our established Community Services Pharmacy Team, a team of skilled and patient-focused clinical pharmacists and medicines management technicians. We provide a clinical pharmacy service to community hospital adult inpatients as well as a medicines optimisation service to patients with complex medication needs in their own homes. This role will be in and around Exmouth and will be based primarily in Exmouth Community Hospital. You will work with colleagues to enable the safe and effective use of medicines through provision of medicines optimisation and personalised medication reviews.

Employees of the Trust benefit from using a shared electronic prescribing and medication administration record (EPIC) which supports integrated care across acute and community services.

* will work with pharmacy technicians to support safe and timely discharge of patients and manage a caseload of referred patients requiring an intervention at home.
* are flexible, organised and committed to delivering patient-centred care.
* can prioritise workload effectively in response to changing patient needs.

You will be offered appropriate training with plenty of scope to develop in the role, with the expectation to qualify and use independent prescribing skills to facilitate excellent patient-focused care while promoting medicines safety, admission avoidance and discharge facilitation.

You will join a self-motivated and flexible team able to work both independently and as part of a team, helping patients regain independence with their medication regimens and stay healthy when they need it most.

The candidate will provide a competent and efficient pharmaceutical service to hospital inpatients and patients at home to meet changing operational demands. Ideally you will have completed, or be on your way towards finishing your clinical diploma and have experience in a hospital pharmacy setting.
