Senior Primary Care Mental Health Practitioner

Full Time - 37.5 hours per week - There is an option to work these hours over 4 or 5 days per week.

EPUT is undertaking a substantial transformation within our community mental health services, supporting the NHS 5 Year Forward Plan and increased mental health funding.
This full-time role offers flexibility to work over 4 days, promoting work-life balance.
Key Responsibilities:
1. Provide seamless services to support patient recovery and well-being in the community.
2. Collaborate with GPs, primary care teams, and consultant psychiatrists to ensure appropriate clinical support from a multidisciplinary perspective.
3. Maintain safe clinical practices, adhering to Care Quality Commission standards and NHS Constitution.
4. Triage patients to facilitate appropriate support, avoiding unnecessary system navigation.
5. Deliver effective, safe services supporting clinical and practice governance.
6. Support team improvement initiatives, safeguarding high standards of care, and fostering an environment of excellence.
7. Conduct physical health investigations, including BMI, blood pressure, and blood tests as appropriate.
8. Provide educational support and training to the primary care team on mental health and well-being.
9. Offer clinical leadership in multidisciplinary meetings, including referrals, reviews, and safeguarding.
10. Adhere to trust policies, contribute to policy development, and ensure professional accountability.
11. Identify, assess, and manage risks through appropriate risk assessments and treatment plans.
12. Administer medications (for nurse applicants), monitor side effects, and review medication effectiveness.
13. Lead and participate in care planning, formulation, and review according to NICE guidelines.
14. Develop relationships with statutory and non-statutory agencies involved in physical and mental health care.
15. Support the development and implementation of operational and clinical policies.
16. Deliver training to other professionals as appropriate.
17. Complete accurate statistics and reports, ensuring KPIs are met.
18. Identify service improvements and implement changes collaboratively.
19. Perform complex nursing assessments and ensure performance targets are achieved.
20. Utilize computer systems effectively for communication and record-keeping.
21. Educate patients and relatives to promote dignity, independence, and quality of life.
