The Inpatient Haematology Ward is pleased to announce the opportunity for a dynamic, organised and proactive Band 3 Discharge co‑ordinator to come and join our team. As the successful post holder you will be expected to work closely with the nursing staff and wider MDT including Discharge Team, Palliative Care Team, Social Services, Care Agencies and Care/Nursing Homes to facilitate the safe and effective discharge of our patients. You will gain experience in managing complex discharges, safeguarding, dealing with challenging situations and communicating with outside agencies.

Recruitment and Work Location
Staff recruited are expected to be available to work flexibly across all our Trust locations to meet service demands. This advert may be closed early if sufficient applications are received. Please note if a role or service relocates as part of a planned move, excess mileage will not be reimbursed. Travel from home to the new work base will be classed as a normal commute. Any other changes will be managed under relevant Trust or national terms and conditions.
