As a practice nurse you will play a crucial role inproviding nursing care and treatment within our practice workingalongside our clinicians.
The role encompasses a wide range of responsibilities, frommanaging chronic conditions to providing health education and screening.

Job responsibilities
The main duties include but are not limited to:
Administering vaccinations and immunisations in line with national and practice guidelines
Cervical cytology
Managing and monitoring patients with chronic diseases (e.g. asthma, COPD, diabetes, hypertension) in line with national and localguidelines
Carrying out home visits for housebound patients
Supporting the clinical team in the management of the carehomes
Ensuring accurate and timely clinical record keeping and documentation
Conducting contraception reviews and coil checks
Carrying new patient and NHS health checks
Wound care and management including dressing, removal of sutures and clips
Participating in infection control, cold chain maintenance, ensuring fridge temperatures are regularly monitored and maintained
Supporting patients with lifestyle advice and self-caremanagement
Advising patients on health care, minor ailments, healthpromotion, and illness prevention
Contributing to achievement of QOF (Quality and Outcomes Framework) and local enhanced services achievement and clinical audits
Preparing and maintaining clinical rooms and equipment before,during and after patient care activities, ensuring a clean and safeenvironment
Ensuring emergency medication and resuscitation equipment is adequately stocked andfit for purpose
Ensuring proper handling and dispatch of laboratory specimens including blood and urine samples
Participating in practice meetings and contributing to our aimfor continuous improvement
Supporting the training and mentoring of junior staff members and HCAs
Person Specification
Experience
* Experience of working within a nursing team in primary care
* Excellent communication and interpersonal skills, and be confident to deal with patients, families and carers
* Ability to work independently and as part of a multidisciplinary team, using initiative within the level of competence for the position
* Commitment to patient confidentiality and data protection
* Accuracy and a strong desire for attention to detail
* Commitment to continuing professional development
* Working knowledge of SystmOne clinical system
* Stable career history showing clear commitment and progression
Qualifications
* Registered General Nurse with current NMC registration
* Qualified & competent in cervical cytology, immunisations, and chronic disease management and phlebotomy
