We are looking to recruit a practice nurse to join our dynamic, progressive nursing team. This role is part time, although the hours are negotiable.

We are a large GP practice in Chippenham Wiltshire, with a patient list size of around 16000.

Our Practice Nurse Team offer chronic disease management, baby immunisations, routine treatment room work, leg club attendance, general healthcare clinics, women's health, and support our staff to excel in their field.

We are a training practice and have the ability, drive and resource to grow and develop all our staff.

Main duties of the job

Main duties include routine health screening, chronic disease management, wound management, female health, smear taking, baby imms, and the ability to work well within a team.

About us

As mentioned before we are a dynamic progressive surgery based in Chippenham. We work very closely with other local surgeries within our PCN to offer joined up care for our patients.

We operate an on the day duty system, and pre-bookable routine appointments. We offer a combination of telephone, video and face to face appointments to meet the varying needs of our patients.

We have a large purpose built building that also accommodates dentists and the out of hours service.

Job responsibilities

Job Summary

The post holder is a registered nurse who acting within their ownprofessional boundaries will provide general practice nursing to the whole ofthe Practice population.

As an autonomous practitioner the nurse is responsiblefor the care delivered, demonstrating critical thinking and skills in clinicaldecision making in the management of patients. They will work collaborativelywithin the general practice team to meet the needs of patients and support thedelivery of policy and procedures.

Key Responsibilities

Clinical Practice

o Assess,plan, develop, implement and evaluate programmes to promote health andwell-being.

o Assess,plan, develop, implement and evaluate individual treatment plans for patientswith asthma and/or COPD.

o Proactively identify,diagnose and manage treatment plans for patients at risk of developing a longterm condition as appropriate

o Work with other health careprofessionals to diagnose, monitor and treat long term conditions, includingnon-drug based treatment methods using a management plan and in line withnational and local policies andpractice needs

o Work with patients in orderto support adherence to prescribed treatments.

o Prioritise health problemsand intervene appropriately to assist the patient in complex, urgent oremergency situations, including initiation of effective emergency care.

o Cervical cytologyconsultations.

o Implement and participate invaccination and immunisation programmes for both adults and children

o Advise, support andadminister vaccinations to patients travelling abroad

o Promote and deliverevidence-based care to patients presenting with aural conditions

o Meet the needs of patientspresenting for opportunistic wound care

Communication

o Utilise and demonstratesensitive communication styles to ensure patients are fully informed andconsent to treatment

o Communicate effectively withpatients and carers, recognising the need for alternative methods ofcommunication to overcome different levels of understanding, culturalbackgrounds and preferred ways of communicating.

o Anticipate barriers tocommunication and take action to remove or reduce those barriers

o Act as an advocate whenrepresenting the patients and colleagues viewpoints to others

Delivering a Quality Service

o Recognise and work withinown competence and professional scope of conduct as regulated by the Nursingand Midwifery Council (NMC)

o Produce accurate, contemporaneousand complete records of patient consultations, consistent with legislation,policies and procedures

o Prioritise, organise andmanage own workload in a manner that maintains and promotes quality

o Deliver care according tothe NSF and National Institute for Clinical Excellence (NICE) guidelines andevidence based care

o Assess effectiveness of caredelivery through self and peer review, bench marking and formal evaluation

o Participate in themaintenance of quality governance systems and processes across theorganisation and its activities

o In partnership with otherclinical teams, collaborate on improving the quality of healthcare,responding to local and national policies and initiatives as appropriate

o Evaluate the patientsresponse to health care provision and the effectiveness of care

o Support and participate inshared learning across the practice and wider organisation

o Participate in themanagement, review and identify learning from patient complaints, clinicalincidents and near-miss events utilising a structured framework.

o Participate in themonitoring review of the team, providing feedback as appropriate

o Understand and apply legalpolicy that supports the identification of vulnerable and abused children andadults, being aware of statutory child/vulnerable adult health procedure andlocal guidance

Personal and PeopleDevelopment

o Take responsibility for owndevelopment, learning and performance including participating in clinicalsupervision and acting as a positive role model

o Support the development ofothers in order to maximise potential

o Actively promote thePractice as a learning environment, encouraging everyone to learn from eachother and from external good practice

o Encourage others to makerealistic self-assessment of their application of knowledge and skills,challenging complacency and actions that are not in the interest of thepublic and/or service users

o Understand ownresponsibilities and accountability in the delivery of GP services topatients, ensuring that the needs of the patient are the priority

o Participate in planning andimplementing changes within the area of care and responsibility

o Critically evaluate andreview innovations and developments that are relevant to own practice

o Keep up-to-date with newdevelopments locally and nationally identifying those that will enhance theteams work

o Ensure awareness of sourcesof support and guidance (e.g. patient advice and liaison service PALS) andprovide information in an acceptable format to all patients, recognising anydifficulties and referring where appropriate

o Promote the role of the GPNin the provision of health care

Team Working

o Understand own role andscope in the organisation and identify how this may develop over time

o Work as an effective andresponsible team member, supporting others and exploring the mechanisms todevelop new ways of working

o Delegate clearly andappropriately, adopting the principles of safe practice and assessment ofcompetence of those taking on delegated duties

o Ensure clear understandingand utilisation of referral mechanisms within the practice

o Accept delegation from othernurses, prioritise own workload and ensure effective time-managementstrategies are embedded in own practice

o Work effectively with othersto clearly define values, direction and policies impacting on care delivery

o Participate in teamactivities that create opportunities to improve patient care

o Participate in and supportlocal projects as agreed with the Practice Management team

Qualifications

* Must hold a current NMC registration
* Chronic Disease qualification for Asthma and COPD.
