About the Role

Hoki ki te Kāinga is an Early Supported Discharge service that provides relationship-centred care to older adults following illness, injury, or surgery.

In this role, you will work under the guidance of registered health professionals to deliver restorative rehabilitation and daily living support. This includes helping people with walking, showering, dressing, preparing meals, and carrying out prescribed exercise programmes. Your support will empower individuals to return to their previous level of function and confidence.

You will work rostered shifts that rotate across mornings (7:30 AM 4:00 PM) and afternoons (11:00 AM 7:30 PM), Monday to Sunday, including public holidays. Rosters are provided monthly, with no set days or fixed shifts - shift are rostered across Napier and Hastings sites.

You’ll also be working closely with whānau, supporting them to play an active role in their loved ones recovery journey.

Hoki ki te Kāinga is a dedicated early discharge and rehabilitation team focused on supporting people to recover in a setting that is familiar, meaningful, and culturally appropriatetheir own home.

We work with older adults across the community, ensuring that their transition from hospital is smooth and that they receive the right support at the right time. Our team is collaborative, values-driven, and committed to delivering care that is person- and whānau-centred.

About you

Youre someone who thrives in a people-focused role and understands the importance of patience, empathy, and respectful communication. Whether youve worked in the health and disability sector before or are looking to begin your career in care, wed love to hear from you.

What were looking for:

* A genuine interest in supporting people to regain their independence
* Confidence to work in peoples homes with sensitivity and professionalism
* Good communication skills and a team mindset
* Willingness to learn and follow guidance from registered health professionals
* Cultural awareness and respect for working with people from diverse backgrounds, including Māori and Pasifika whānau
* A full drivers licence is a must

This role could be an excellent entry point into health and rehabilitation, or a meaningful next step in your support work journey.
