Community Recovery Navigator- Community Services

Location: Severn House Mandale Park Belmont County Durham DH1 1TH
Working Hours: Monday to Thursday 9am-5pm and Friday 9am-4:30pm
Contract Type: Permanent
Salary: £25,110-£32,090

About the Role

To provide support to people residing in County Durham who are being discharged from community hospitals within in the area.

We work in partnership with Home Group to deliver support for people experiencing mental health and wellbeing issues which may be delaying discharge from the hospital. We support people in the community developing personalised support plans to meet their individual needs. The main aim of the service is to reduce reoccurring presentations at both GP practices and A&E departments.


What You’ll Do

1. Support clients to live independently in their own accommodation and progress towards sustained independent housing.
2. Manage a caseload of service users as allocated by the Team Manager and complete high quality and comprehensive needs assessments and risk assessments for those service users.
3. Develop comprehensive support / risk plans to identify how individual service user needs will be met in order to help each service user live independently within the community and achieve sustained outcomes.

To Succeed in This Role, You’ll Need:

4. NVQ Level 2 in Health & Social Care (or above or equivalent) or willing to work towards this in post.
5. Proficient in Microsoft Office programmes
6. Knowledge of welfare benefits system and relevant legislation.
7. Driving licence and access to own vehicle as this role will be working within the community
