Role: Through the Gate Worker
We have an exciting opportunity for a Through the Gate Worker to join our Coventry Team. The successful candidate will have the skills and experience to assess and engage with our service users, ensuring they access the right intervention at the right time, in a way that best meets their needs.
The role involves meeting service users upon their release from prison, providing immediate support on their day of release, and assisting them in attending initial post-release appointments in the community. This includes practical and emotional support to facilitate a smooth transition from custody to the community.

* Full Time Hours: 37.5 per week
* Salary Range: £27,861.26 - £32,002.35 (dependent on experience, pro rata for part-time hours)
* Contract Type: Fixed-term until 31st March 2026
Responsibilities
* Work intensively with patients for 12 weeks prior to and post-release
* Design holistic resettlement plans to support healthcare continuity after release from prison
* Complete secondary screening assessments for patients arriving in prison as part of the Integrated Health and Wellbeing model
* Coordinate with CGL teams, healthcare, prison partners, and external agencies for effective release planning
* Provide advocacy for access to partnership services
* Contribute to risk management processes, including prison ACCT processes
* Support social reintegration, promoting recovery, resilience, peer support, and self-determination
Essential Skills and Experience
* Strong understanding of substance misuse issues and experience within a similar field, ideally within prison/criminal justice
* Excellent verbal and written communication skills, with good IT skills
* Proactive, able to work independently with excellent time management and prioritization skills
* Knowledge of mental health interventions, services, and good practice
* Supportive team player with strong interpersonal skills, capable of working in partnership with professionals and agencies
* Ability to manage change effectively, prioritizing service user needs
* Understanding of information governance and safeguarding procedures
* Willingness to learn and contribute to team development
