This is a fantastic opportunity for an experienced Social Worker to join the Adult Care Management Team, working within a Discharge to Assess (D2A) model while also managing a hybrid caseload of longer-term complex cases. The Role: You’ll work with adults aged 18 to assess needs, support safe hospital discharge where applicable, and ensure effective care planning under the Care Act (2014). This position sits within Team 2 following the D2A model, with opportunities to manage varied and complex cases from across Adult Services. Key Responsibilities: Carry out Care Act-compliant assessments, reviews and support planning. Support safe and timely hospital discharge under the Discharge to Assess model. Lead on S.42 safeguarding enquiries and apply Making Safeguarding Personal. Work collaboratively with Health, internal departments, and partner agencies. Deliver strengths-based, person-centred interventions that promote independence. Maintain clear case recordings, risk assessments and reports. Provide professional guidance to colleagues, partners and carers. Take opportunities to prevent, reduce or delay the need for ongoing services.Requirements: Registered Social Worker with Social Work England. At least 2 years’ post-qualifying experience in a statutory adult social care setting. Knowledge of the Care Act 2014, Mental Capacity Act and safeguarding procedures. Experience working within hospital discharge, D2A or complex adults’ teams is desirable. Ability to manage competing priorities and work to statutory timescales. Strong communication and report-writing skills.
