What you’ll do
* Support customers with daily living, personal care and medication
* Help people build skills and confidence to live more independently
* Co-ordinate rotas and guide support workers to deliver great care
* Keep support plans and risk assessments up to date
* Plan activities and build links in the community

You have
* Level 2 in Health and Social Care or equivalent experience
* Experience supporting people with learning disabilities
* Calm, resilient and able to work on your own initiative
* Great communication skills and a person-centred approach
* Confidence using tech to update plans and complete training
