Home First Clinician – Surrey Downs H&C

The closing date is 14 May 2026

An exciting opportunity has arisen for an experienced Home First Clinician (Nurse or AHP) to join the integrated community team within Surrey Downs Health and Care, hosted by Epsom and St Helier University Hospitals NHS Trust.

Working across the Surrey Downs system, you will deliver urgent community response and short-term care to prevent hospital admissions and support safe, timely discharges from acute settings. As part of a 7‑day Home First Service, you will work within the Transfer of Care Hub, rotating across triage, in‑reach, and care home liaison functions to reduce discharge delays and ensure seamless patient pathways.

You will be a skilled clinician, responsible for assessment, diagnosis, treatment, and evaluation, using sound clinical judgement and decision‑making to manage complex patient needs. Working collaboratively within a multidisciplinary team, you will contribute to service delivery, leadership, and continuous improvement, supporting high‑quality, person‑centred care.

This is an excellent opportunity to further develop your clinical leadership and integrated care experience within a forward‑thinking NHS partnership.

Main duties of the job

* Deliver expert, specialist service for adults with complex health and social needs who may benefit from the Home First Service.
* Coordinate a seamless service through the development of enhanced interdisciplinary team processes and communication within the Home First Service and across the wider Surrey Downs Health and Care whole system.
* Empower patients and carers to make informed decisions regarding their goals and outcomes and the care they will receive to support them to meet these.
* Support the development and evaluation of clinical protocols and systems of inter‑agency documentation to enhance continuity and standards of care.
* Understand and apply the Mental Capacity Act in regard to mental capacity in decision making and appropriate application of Deprivation of Liberty requirements, working with other agencies within the Safeguarding framework.
* Attend meetings as required, including deputising for the Clinical Manager when requested.
* Develop close links with the wider community to enhance both care for individual patients and the wider development of Epson Health and Care.

About us

Surrey Downs Health and Care deliver care closer to people's own communities through our Primary Care Networks, Community Hospitals, Specialist Services and our innovative partnership of local NHS organisations.

Surrey Downs Health and Care has a track record of providing person‑centred care that goes beyond organisational boundaries to do what is best for the individual. This partnership includes:

* The three GP federations GP Health Partners, Dorking Health Care and Surrey Medical Network representing practices that operate in the Surrey Downs area
* Epsom and St Helier's University Hospitals NHS Trust
* Surrey County Council

Historically, there have been boundary lines between the organisations that provide care to people in their homes, in GP surgeries and in hospitals, but we have always been united in our mission to provide great care to the people who need us.

It's on those grounds that the Surrey Downs Health and Care was formed – we want local people to receive the care that they need in the right environment. By bringing together our expertise, we can improve patient care and enable local people to access the right support, care and treatment more easily than ever before.

In bringing this partnership together, we are working to the same set of values that will translate into better care for our residents.
