Who You Are A qualified Social Worker with at least 3 years of experience, familiar with working with adults who have cognitive impairments and physical disabilities. You have a background in settings such as older adults, learning disabilities, or hospital social work. You are capable of managing a diverse caseload independently, including complex cases, while efficiently resolving challenges and managing your time. What the Job Involves Based within the Waltham Forest Older Adults Mental Health Team, this role requires you to collaborate with a multi-disciplinary team. You will support older individuals with severe and enduring mental health conditions, possibly with additional physical health needs. Responsibilities include managing cases involving community care, hospital discharges, safeguarding, and Continuing Health Care (CHC). You will conduct home visits across Waltham Forest and partake in monthly supervision within a supportive team environment. The ability to work from the office in Leyton is required, with some potential for home working. Skills Proficient in Care Act and Adult Safeguarding Understanding of Mental Capacity Act Knowledge of Mental Health Act, including s117 Experience with Continuing Health Care (CHC) Positive attitude towards teamwork and perseverance Ability to travel for home visits Experience in multidisciplinary team within NHS (desirable) Post qualifying experience such as BIA or AMHP (desirable)
