An exciting opportunity has arisen in a community rehabilitation team for a band 6 physiotherapist to join a supportive and enthusiastic community therapy service as a Physiotherapist, working across community settings to deliver rehabilitation in patients' own homes following discharge from an acute bed, or supporting long-term condition management.
The team comprises qualified physiotherapists, assistant practitioners, and assistant therapy staff, who work closely with the wider MDT and INT teams in the community.
You will work with patients with a variety of conditions and disabilities, in their own homes and the local community. Responsibilities include assessment, intervention, and discharge of a caseload of patients, acting as a direct supervisor to staff, and providing support and expertise to team members.
We offer a comprehensive training program within a supportive team environment.
The service operates five days a week, Monday to Friday. Part-time working is considered. Secondment is encouraged—please ensure you have manager’s approval before applying.
The post holder is an autonomous practitioner responsible for specialised assessment and treatment of referred patients, managing a caseload of patients with complex conditions, including long-term management and hospital discharges. You will see patients in their homes, collaborating with MDTs from other organisations. The role includes specialist assessments, treatment, and prescription of essential equipment as part of therapeutic intervention.
You will also serve as an important link with PCN and MDT teams, attending meetings as part of neighbourhood team working.
