SDHC Home First Services supports the approach of “why not home, why not today?” and focuses on reducing hospital attendances through admission avoidance. Wherever possible, patients should be supported at home; when not possible, alternative pathways are available. Staying in hospital longer than necessary negatively impacts patient outcomes.
Surrey Downs Health & Care and Epsom & St Helier University Hospitals Trust have developed an integrated model of care, bringing together primary care, secondary care, and community services within Surrey Downs Place. This model supports patients by offering acute-level care, monitoring, and treatment at home. It provides a safe and efficient alternative to NHS bedded care, preventing avoidable hospital admissions and supporting early discharge. The Home First service operates 7 days a week, from 8 am to 8 pm, delivered by a multidisciplinary team including geriatric consultants, doctors, nurses, therapists, physician associates, paramedics, pharmacists, rehabilitation support workers, and administrative staff. The team offers medical assessment, daily reviews, MDT discussions, and condition management.
Home First encompasses the following functions: Virtual Ward, Urgent Community Response, Front Door, Home First+, Recovery at Home, Mary Seacole Frailty Unit
Main duties of the job
* The post holder will be an experienced healthcare practitioner providing care within their professional boundaries for patients in the UCR and Virtual Ward of the HomeFirst service. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills.
* The post holder will demonstrate critical thinking in clinical decision-making, work collaboratively within the MDT to meet patient needs, support policy and procedure delivery, and provide professional leadership as required. Advanced practice requirements, such as those from the NMC, must be met.
* The ACP will use specialist knowledge and skills to independently provide healthcare to patients. They are accountable for the entire episode of care for patients with undifferentiated and undiagnosed needs.
* The post holder will act as a role model, delivering advanced practice and promoting clinical effectiveness. They will work autonomously in assessments, examinations, diagnosis, decision-making, care planning, prescribing, health promotion, and safe discharge.
* The post holder will work closely with all MDT members and contribute to the operational and strategic development of the service.

Person specification
Qualifications
* Registered first-level nurse/practitioner with a relevant health degree
* Completed Master’s Degree in Advanced Practice or working towards
* Non-medical independent prescribing qualification or working towards
* Advanced clinical practice skills/competencies
* Management of patients with acute medical illnesses
* Clinical examination skills
* In-depth knowledge in their specialist clinical field
Desirable criteria
* Demonstration of leadership or management experience
Experience
* Significant post-registration experience
* Leadership in quality initiatives such as clinical benchmarking and service development
* Experience in audit and research
* Healthcare professional prescribing
