Job Responsibilities

Clinical Care & Patient Management
* Conduct comprehensive physical and psychological assessments to address a wide range of presenting problems.
* Deliver high‑quality consultations through various modalities, including face‑to‑face appointments, telephone triage, home visits, and eConsultations.
* Manage acute and chronic conditions, ensuring all care is evidence‑based and aligned with current clinical protocols.
* Proactively screen for risk factors and early signs of illness, developing personalised care plans for long‑term condition management.
Administrative & Operational Excellence
* Maintain accurate, detailed, and contemporaneous clinical records in line with professional standards.

* Oversee prescribing tasks, including issuing acute and repeat prescriptions in accordance with the practice formulary.
