Overview
We are seeking a dynamic Band 7 Clinical Specialist Physiotherapist to work across Wrexham Community Services and Chirk Community Hospital, delivering high‑quality, community‑focused physiotherapy care.
In this role you will work autonomously, providing expert assessment, diagnosis and evidence‑based intervention for adults with complex, multi‑pathology needs across a variety of community settings. You will play a key role in shaping and improving community physiotherapy services across Wrexham, helping to enhance accessibility, improve rehabilitation pathways and deliver high‑quality, person‑centred care closer to home.
You will provide clinical leadership across the Wrexham Domiciliary Team, Community Resource Team (CRT), Integrated Prevention Service (IPS), Reablement, Home First, and Chirk Community Hospital. Working closely with multidisciplinary colleagues and fellow Clinical Specialists, you will help deliver an innovative and responsive rehabilitation service. You will also support service development, supervise junior staff and students, and contribute to quality improvement initiatives.
We are seeking someone who demonstrates advanced clinical reasoning, confidence in managing complex caseloads, strong leadership and communication skills, and a passion for collaborative MDT working and community‑based care. Key Duties
* Work collaboratively with the Advanced Practitioner and Physiotherapy leadership team to deliver a high‑quality clinical service in the East Integrated Health Community.
* Assess and manage complex patients, developing and delivering individualised treatment programmes and working effectively with the wider multidisciplinary team.
* Provide clinical leadership and day‑to‑day management of the service, including planning and coordinating service delivery and evaluating services once competencies are completed.
* Collaborate with Falls leads across BCUHB to develop service improvement opportunities.
* Support the ongoing education and development of the team to ensure a high‑quality, evidence‑based service.
* Act as lead educator for student placements.
The ability to speak Welsh is desirable; English and/or Welsh speakers are encouraged to apply.
