About the role:
1. Supporting service users in Aftercare/Recovery Support through their remaining time in treatment with the service.
2. Providing 1-1 sessions and structured group work programme and recovery planning and onward referrals to specialist services.
3. Reducing drug and alcohol related harm to service users and the wider community
4. Work with prescribing clinicians to support substitute prescribing regimes to improve social functioning.
5. Aftercare care coordinator overseeing assigned groups of service users, providing: 1-1s; structured group work; asset mapping; recovery planning (using motivational interviewing techniques) and reviews of care.
About you:
6. Excellent understanding of drug and alcohol issues and experience of working within a related field.
7. Accomplished written and verbal communication skills and a high degree of personal IT competency.
8. An understanding of and commitment to safeguarding best practice.
9. Effective interpersonal skills with the ability to engage successfully with internal and external stakeholders at all levels.
10. Excellent time management skills, and an ability to work on own initiative, prioritisingaccordingly.
