Social Work Professional - Hospital Discharge & Care Management

Location: Oban, Argyll and Bute

Rate: £38.00 - £40.00 per hour

Duration: Immediate Start - End of June 2026

Working Pattern: Full-time (On-site with occasional hybrid flexibility)

Are you an experienced social care professional looking to make a meaningful impact in a stunning coastal location? We are seeking a dedicated and knowledgeable individual to join our team in Oban, supporting vital hospital-based social work and care management.

This is a fast-paced, rewarding role that puts you at the heart of the community, ensuring patients receive the care and protection they need as they transition back to their homes.

The Role

Working within the unique landscape of Argyll and Bute, you will manage a varied caseload focused on assessments and care management. While hospital-based experience is a plus, your expertise in Scottish social work legislation is the true priority.

Key Responsibilities:

* Conducting comprehensive assessments and managing complex care packages.
* Applying deep knowledge of Social Work Scotland frameworks.
* Ensuring compliance and advocacy under the Adults with Incapacity (AWI) Act and the Adult Support and Protection (ASP) Act.
* Navigating the Capacity Act to ensure patient rights and safety are upheld.

Requirements

We are looking for someone who can hit the ground running. To be successful, you must have:

* Extensive experience with ASP, AWI, and the Capacity Act.
* A background in Social Work Scotland standards and care management.
* The ability to work full-time in Oban (non-hybrid to start).
* Strong communication skills to collaborate with multi-disciplinary hospital teams.
