Job Summary
We have an exciting opportunity for a community CNS position at Meadow House Hospice to help deliver the new North West London model of specialist palliative care. The model increases visiting hours and improves access to timely care for residents of Ealing and Hounslow.
Patients present with complex, specialist needs, with a shift from curative to palliative focus. The hospice provides a 15‑bed inpatient unit, allied health professionals, and a large community team.
Key Responsibilities
* Deliver high‑quality community palliative care and support for patients and families.
* Assess patient needs, obtain history, and manage a caseload of community patients.
* Exercise professional autonomy and make critical decisions to meet patient needs.
* Collaborate closely with primary care teams, communicating effectively and building relationships.
* Advise and guide patients and carers on palliative care options and support.
Required Skills & Experience
* Strong clinical background and experience in palliative or community nursing.
* Demonstrated ability to work collaboratively with multidisciplinary healthcare teams.
* Excellent communication and interpersonal skills.
* Motivated, dynamic, and committed to improving patient outcomes.
* Ability to manage complex patient care with compassion and efficiency.
