Overview
Part-time Practice Nurse role at Helmsley Medical Centre and Terrington Surgery, due to planned retirement. Open to experienced practice nurses, newly qualified nurses or those new to general practice. A year of supported teaching and learning is available through the General Practice Nurse vocational training scheme. Applicants from community nursing or secondary care settings are welcomed.
This is an exciting development opportunity to join an enthusiastic and committed team. The Practice Nurse will deliver nursing services to the practice population, supported by Senior Nurses, focusing on long-term condition management, health promotion, education and screening activities.
Responsibilities
* Meet patients’ nursing care needs in the practice; some home visits may be required. Assess, plan, provide and evaluate nursing care within scope of competence.
* Collaborate with the multidisciplinary team to meet patient needs and support key performance indicators for Helmsley and Terrington Practices.
* Provide general nursing care, including wound management and chronic disease management; vaccinations and immunisations for babies, children and adults.
* Deliver women’s health services including cervical screening and contraception; conduct health assessments and screenings.
* Support patients with long-term conditions through education and self-management advice; participate in health promotion activities.
* Maintain accurate and up-to-date patient records; participate in safeguarding and governance processes.
* Co-ordinate travel medicine services; request and process pathology results as required.
* Oversee chronic disease clinics, refer to secondary/specialist care when needed, and ensure appropriate use of chronic disease registers.
* Participate in well-woman and well-man clinics and provide routine nursing care as appropriate; assist GPs with minor surgery when required.
* Prioritise health issues and intervene appropriately; support the clinical team with emergencies.
* Contribute to health promotion and education; liaise with external services to support vulnerable patients.
Secondary Responsibilities
* Participate in local initiatives to enhance service delivery and patient care; support shared learning within the practice.
* Review clinical practices in light of national policies and initiatives; participate in significant event reviews when applicable.
Qualifications and Experience
Essential
* Registered Nurse (NMC)
* Experience of working autonomously
* Experience of infection prevention and control measures
* Ability to work within own scope of practice; know when to refer to GPs
* Understanding of evidence-based practice and necessity of PGDs; ability to record accurate clinical notes
Desirable
* Postgraduate qualification in chronic disease management, mentor/teaching qualification, family planning qualification
* Clinical knowledge in wound care, ECGs, venepuncture, travel medicine, immunisations, well-woman/well-man clinics, and pathology testing
* Broad knowledge of clinical governance and local public health issues

Other Requirements
* Flexibility to work outside core hours; DBS check; Occupational Health clearance
* Meet requirements for nurse revalidation; CPD evidence
* Current UK professional registration

* Contract: Permanent; Part-time (17 hours per week across two sites with rota to be agreed)
* Salary: Band 5 depending on experience
