Join us for an amazing opportunity as an experienced Social Worker in our dynamic Living Well Team in the bustling town of Reading. You will collaborate with an Occupational Therapist and commissioning colleagues to provide stellar support to service users accessing Supported Living services. This position is full-time and offers a rewarding hourly rate of £38. Over the course of 3 months, you’ll contribute to maximizing the independence of our service users, ensuring their support is perfectly tailored to their needs.
Responsibilities
* Conduct assessments and reviews for adults in supported living, ensuring care packages are tailored to meet individual needs.
* Collaborate with providers to ensure commissioned hours are appropriate and resources offer value for money.
* Optimize funding resources through effective case management and the development of personalized support plans that promote independence and wellbeing.
* Work closely with multi-disciplinary teams, service providers, and service users to achieve positive outcomes.
* Engage in regular on-site presence at the Civic Offices, Reading for essential meetings and site visits, while balancing remote work throughout the week.
* Previous experience in a supported living context.
* Proven ability to deliver cost savings through outcome-focused practice.
* Strong skills in assessment, review, and care-planning.
* Expertise in collaborating effectively with multi-disciplinary teams, providers, and service users.
