Adults Mental Health Social Worker
ESSENTIAL:
* Qualified Social Worker with evidence of relevant qualifications
* Minimum of 5 years post qualifying experience
* Registered with Social Work England
* Experience of working in an Adult Social Care statutory setting
* Knowledge and experience of working with people who have severe and enduring mental ill-health
* Experience of undertaking assessments of need, scheduled and unscheduled reviews, creating care plans, and commissioning support to meet needs.
* Experience of statutory safeguarding under s42(1) and s42(2) Care Act
* Experience of completing Mental Capacity Assessments
* Knowledge of the Care Act, Mental Capacity Act, Mental Health Act, Equalities Act, Human Rights Act, Data protection, GDPR, Continuing Health Care
* Knowledge of Deprivation of Liberty Safeguards
* 2 references

DESIRABLE:
* Experience of completing social circumstance reports for Tribunals and Managers Hearings

The Job:
Agency Social Worker opportunities are available within the Mental Health Review Team in Middleton and CMHT West in Rochdale. The role involves undertaking social work statutory functions within an Adult Care mental health service. The team is co-located with health colleagues and operates in a multi-disciplinary manner.
Responsibilities include conducting Strength Based Needs assessments, reviewing care packages, updating care plans, undertaking mental capacity assessments, safeguarding work, and facilitating hospital discharges for individuals with severe and enduring mental health diagnoses, who are under secondary mental health services. Knowledge of mental health pathways is beneficial.
WORKING PATTERN:
37 hours per week, Monday to Friday. There is some scope for remote work with manager approval, but the role is primarily office-based due to client needs.
