The Wokingham Older Adult Mental Health Service is looking for an enthusiastic, caring and highly motivated Community Support Worker to join our friendly and innovative Home Treatment Team. We are based at Wokingham Hospital where we have access to free parking and an onsite café. We are friendly and supportive team with a shared passion for providing our patients and carers with the best experience of using our service.
Your role will be primarily within the Home Treatment Team however your community caseload will cover the wider service, working with the qualified mental health practitioners in line with service needs. This may include group work supporting cognitive stimulation therapy and our living well with dementia groups as well as supporting anyadditionalfuture groups asrequired.
The Wokingham Community Older Adult Mental Health Service a multi-disciplinary team with specialist skills in caring for older people with mental health needs in the community. The post holder will under supervision hold a caseload of patients with the aim of maximising independence and improving wellbeing. In addition, the post holder will support the delivery of excellent care provided to our HTT caseload,providing high quality and intensive mental health interventions inthe homes of people with functional mental health problems, as well as people with dementia.

The “must haves” for this role:
• Have recent experience of supporting older adults with functional mental health needs including dementia.
• Be able to work shifts -a combination of 9-5pm or 11-7pm, including one weekend 9-3pm once a month
• You will be a team player, be highly motived, compassionate and demonstrate a good understanding of older adults with mental health needs.
• Hold a valid UK driving licence and have access to a car for work purposes, including carrying patients and carers as passengers
