We are seeking a qualified Practice Nurse, ideally with experience in a GP setting, to join our dedicated and supportive team.
As a valued member of the practice, you will work collaboratively alongside GPs, Nurses, Paramedics, and other healthcare professionals to deliver high-quality, patient-centred care.

Main duties of the job
The post holder will play a key role in the management ofpatients across the practice population, delivering proactive, patient-centredcare and supporting QOF and IIF outcomes.

Job responsibilities
Provision of childhood and adultimmunisations.
Provision of cervical smears.
Promoting health education andself-care
Providing travel health adviceand immunisations
Contributing to QOF, enhancedservices, and practice development initiatives
Work within the SystmOne Clinicalsystem to record consultations, manage recalls, and contribute to clinicalaudits.
Collaborate with GPs,pharmacists, practice nurses, HCAs and external services.
Support the practice in meetingQOF, IIF, and other long-term condition targets.
Provide ongoing education andlifestyle advice to empower patients in self-management.
Support service development andaudit quality improvement initiatives.
Person Specification
Qualifications
* Registered Nurse with a valid NMC registration.
* Experience in delivering vaccinations and cervical screening (smears) within primary or community care.
* Excellent communication, time management, and patient education skills, with a focus on empowering patients.
* Able to work independently while contributing effectively within a multidisciplinary team.
* Compassionate, proactive, and patient-centred approach to care.
* Willingness to undertake training in chronic disease management where required.
* Knowledge and experience of SystmOne.
* Experience in travel health and vaccinations desirable.
* Confidence in managing some or all chronic disease areas.
