Primary Care Network Clinical Pharmacist
West Enfield Collaborative Primary Care Network has an exciting opportunity for a Practice Pharmacist to join its PCN based in North London, providing high-quality care to around 40,000 patients. The PCN covers three GP sites, two of which are approved training practices. We are interested in candidates who are already fully functioning Pharmacists seeking an opportunity to broaden their horizons, gain more autonomy, and take on additional clinical responsibilities.
Your role will involve participating in all aspects of patient care, utilizing knowledge gained from your training.
Main duties of the job
Support the partners in implementing effective medicine management within the practice, identify areas for improvement, and initiate and manage change. Collaborate with an allied healthcare professional team including Pharmacists, Paramedics, Physician Associates, Nurses, and Healthcare Assistants, with support and supervision from a GP.

We are a PCN in North London (Enfield), providing high-quality care to approximately 40,000 patients. The PCN covers three well-established GP sites, two of which are approved training practices. We value collaborative working and hold regular team meetings. Our workforce includes GPs, GP and FY2 trainees, Nurses, Pharmacists, Social Prescribers, Care Coordinators, and administrative staff. You will be employed by the PCN, with your time divided between one or more sites based on hours worked and PCN needs.
Job responsibilities
* Develop and manage a medicines management plan and deliver patient services in line with practice policies, and local and national guidance.
* Provide expertise in clinical medicines review and address the public health and social needs of patients in GP practices.
* Reduce inappropriate polypharmacy and wasteful prescribing through clinical medication review.
* Reconcile medicines following hospital discharge and collaborate with patients and community pharmacists to ensure proper medication management post-discharge.
* Contribute to reducing medicine-related hospital admissions and readmissions by supporting optimal medication use and addressing medication-related issues.
* Run clinics focused on medicines, such as anticoagulant monitoring clinics.
Contribute to multi-morbidity reviews and multidisciplinary reviews. Undertake minor ailments clinics and triage.
