Job overview An opportunity for an experienced and enthusiastic Specialist Stroke Nurse to join a well-established Community Stroke Service (CSS) The team delivers both Early Supported Discharge and Community Stroke rehabilitation for up to 12 weeks following acute stroke We have a full Multi-disciplinary team including, Physiotherapy, Occupational Therapy, Clinical Psychology, Speech and Language Therapy, Rehabilitation Assistants and care provided by Active Recovery Assistants. About Us The Community Stroke Service is an important part of the Sheffield Stroke Pathway facilitating discharge from both acute wards and the Stroke Specialist Rehabilitation bedded (SPARC) unit We also receive community referrals from GPs and the Stroke clinics CSS are part of the Integrated and Geriatric Stroke Medicine Directorate (IGSM) under the Combined Community and Acute Care Group (CCAG). Advert Who we are looking for The stroke team is constantly looking at ways to develop and improve the service. Having an understanding of service improvement would be an advantage to enable you to contribute and possibly take a lead in this important area This role requires someone with a very flexible approach as it is an extremely varied role involving acute care, preventative input and educational elements The nurses assess, plan, co-ordinate and deliver nursing interventions, and in addition to contribute to the holistic assessment and care planning for CSS patients Duties include integrating the nursing role with the other disciplines in the community stroke Team.
