Job Summary
To support Patients with palliative or end of life prognosis and their families/carers to achieve their preferred place of care. To work under the supervision of Clinical nurse Specialists to develop knowledge and expertise in Palliative care whilst providing a timely and service. Assisting Clinical Nurse Specialists to manage a caseload of patients supporting with telephone calls, home visits, bereavement support and symptom management. To provide clinical leadership to junior members of the team with support, teaching, and clinical guidance. The RGN will be expected, under supervision, to deliver high standard of personalised care and identify needs of the patients through holistic assessment. To use their experience and knowledge of palliative care to deliver appropriate symptom control, clinical and personal care, and medication administration. To work collaboratively with the Community Specialist Palliative Care Team (CSPCT), GPs and district nursing teams. To be skilled in communication. To be responsible, support and provide clinical leadership to junior members of staff and Student Nurses. To be aware of cost effective and efficient use of resources. To provide supportive care to patients, their families, and carers. To monitor patients on an ongoing basis, to evaluate outcomes and adjust care plans to ensure optimum co‑ordination and delivery of care. Assist the community clinical nurse specialists with visiting and telephone contact to patients/families as appropriate. Advise on and facilitate bereavement support for bereaved relatives. Undertake bereavement visits as appropriate. To develop and maintain effective relationships with all members of the multi‑disciplinary team to ensure collaborative working for the benefit of patient care. To support the development and implementation of care pathways and patient information. To maintain accurate and up‑to‑date patient records (including electronic records). To provide a high standard of personalised and on‑going care for bereaved families.
