Job Summary
Greater Manchester ICB – Bury locality operates a Continuing Healthcare/Complex case service on behalf of the registered population Bury. The post holder will be responsible for the case management of a caseload of individuals with complex mental health needs. The caseload will comprise individuals with complex mental health needs who have been assessed under the Mental Health Act, along with others who require an individually commissioned package of support. The post holder will be required to make recommendations regarding eligibility for funding.
The post holder will lead and coordinate key professionals across Mental Health services, Local Authority and the Independent Sector to ensure that effective commissioning of service provision occurs and that once established, is monitored and reviewed to deliver measurable goals and outcomes for the service user and GM ICB – Bury.
The post holder will take a lead role in the identification of suitable care providers and will be responsible for ongoing review and reassessment of patients and commissioned packages of care. This will be with a view to determining continued eligibility and ensuring that the package of care continues to meet assessed needs.
Responsibilities
* Commissioning, monitoring and reviewing individualised packages of support to ensure they continue to meet need and provide best value.
* Liaising with patients, families and carers regarding individualised commissioning and review; promoting service user involvement and collaborative working with other agencies.
* Acting as a resource for staff undertaking clinical assessments, providing guidance and supervision to ensure a consistent, informed approach compliant with national policy.
* Conducting independent expert clinical assessment of an individual service user with nursing or highly complex healthcare needs where necessary.
* Supporting the Head of Service by providing expert advice regarding eligibility for funding and attending Funding Panels with scope of practice.
* Negotiating with care providers, both NHS and external agencies, on how care will be delivered to complex, intense care needs service users and applying for funding as required.
* Attending Adult Safeguarding strategy meetings where required, representing NHS Bury Integrated Care System.
* Ensuring all service users eligible for health funding are reviewed in accordance with guidance and taking appropriate steps to ensure service providers adhere to contractual agreements.
* Providing clinical evidence and advice in respect of court proceedings (court of protection, criminal prosecutions, etc.) when requested.
Person Specification
Essential Qualifications
* First degree or equivalent professional development through accredited learning.
* Registered mental health nurse.
Essential Experience
* Significant relevant experience.
* Experience of working with complex cases.
* Experience of case management.
* Able to demonstrate extensive knowledge of the Mental Health Act and associated policy, guidance, legislation.
* Ability to provide and receive highly complex, sensitive or contentious information, negotiate with other professionals and families where resistance may exist.
Additional Requirements
* Subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 – applicant must complete a Disclosure and Barring Service check.
