We are seeking to recruit two experienced and motivated Band 6 Senior Mental Health Practitioners with significant post qualifying experience to join our dynamic team in the Torbay Liaison Psychiatry Service. Job Responsibilities
* The Liaison Psychiatry Service provides a 24 hour, 7 day a week in and out of hours comprehensive service to patients, clinicians and carers at the local District General Hospital (DGH). This includes specialist mental health assessment and intervention to people of all ages from the age of 18 who present or are admitted to the DGH.
* We provide a specialist consultation-liaison service to referring clinicians and clinical teams, assisting them in decision making in difficult and complex cases, and providing both formal and informal education to qualified and unqualified staff, and offer a broad range of specialist psychosocial treatments for individual inpatients.
* Staff represent the Devon Partnership Trust in the general hospital setting and play a vital role in communicating and maintaining a good working relationship between the two organisations.
* Through consultation, assessment and interventions, the Liaison Psychiatry Service aims to: arrive at a formulation identifying the correct care pathway; improve quality of mental health care and quality of life; identify and manage risk; reduce hospital length of stay and admissions; reduce incidents and complaints; measure health outcome improvements; determine service needs.
* A key aim is to contribute to the acute care pathway, ensuring timely specialist mental health assessment, including presentations with self-harm and suicidal behaviours.
* In Torbay, the service covers Acute Care Pathway (ACP), Inpatient care, and Integrated Psychological Medicine Service (IPMS) to provide access to the right treatment at the right time for those with complex physical and psychological needs.
Duties and Responsibilities
* Communication and Working Relationship Skills: contribute to the team, engage with staff, service users, carers and other agencies; impart complex information to patients and families; liaise with primary care and external agencies; present information to patients and families.
* Planning and Organisational Skills: attend meetings; plan and coordinate own workload; organise case conferences with other agencies.
* Analytical and Judgemental Skills: undertake bio-psycho-social assessments; develop formulations and risk management plans; adjust care pathways as needed; seek supervision when necessary.
* Research and Development: participate in research and development aligned to service delivery and development.
* Freedom to Act: work without direct supervision; supervise and delegate appropriately; adhere to policies.
* Patient/Client Care, Treatment and Therapy: develop care plans for in-DGH service users; involve service users and carers; triage referrals; provide specialist advice; participate in safeguarding; apply evidence-based practices.
* Policy and Service Development Implementation: participate in audits and clinical governance; contribute to policy development; engage in networking and benchmarking activities.
* Human Resources: supervision of junior staff; provide clinical supervision; coordinate team activity; mentor students; participate in appraisals and staff support; deliver training; maintain Core and Essential Training.
* Information Resources and Administrative Duties: maintain up-to-date clinical records in line with procedures.
* Other Task Requirements: comply with core competencies for mental health liaison practitioners as described in the Competency Framework for Liaison Mental Health Nursing.
Person Specification
Qualifications Essential: a professional mental health qualification (e.g., RMN, CQSW/ASW, BSc/Diploma in OT) and current registration with a professional body (e.g., NMC). Desirable: postgraduate qualification relevant to specialism; clinical supervision qualification; mentorship qualification.
Experience Essential: experience of working in a multi-disciplinary and multi-agency environment; assessment experience (mental health & risk). Desirable: experience in community mental health, user involvement, and primary care settings.
Other Requirements
