EHSCP Assistant Practitioner Band 4 – Intermediate Care Bed‑Based Rehabilitation
Overview
This role is an exciting opportunity to join the Edinburgh Health and Social Care Partnership and develop community rehabilitation services, including bed‑based rehabilitation within our community hospitals. The post holder will work within an intermediate care multi‑disciplinary team at Ellens Glen Community Hospital and Findlay Community Hospital.
Key Responsibilities
* Work as part of the AHP team alongside the Multi‑Disciplinary Team to plan and coordinate discharge planning activities for frail older people admitted to the hospitals.
* Plan and coordinate bed‑based intermediate care services within the community hospitals.
* Collaborate with multidisciplinary team members at all stages of rehabilitation and discharge.
* Support patients and families throughout the discharge and recovery process.
Requirements
* Professional registration suitable for an Assistant Practitioner role (e.g., physiotherapy, occupational therapy, speech and language therapy, or allied health professional).
* Valid PVG Scheme membership or the ability to obtain one before commencing employment.
* Successful criminal records check as required by the PVG Scheme.
* Experience working with frail older adults in a hospital or community setting is desirable.
* Strong communication and collaborative skills.

Additional Information
Full‑time working week will be reduced from 37 to 36 hours per week as of 1/4/26, with a corresponding increase in hourly rate to ensure total pay is not affected. Part‑time hours will be reduced pro‑rata.
Employment is based in the Edinburgh Health and Social Care Partnership community hospitals.
