Specialist Rehabilitation Physiotherapist
Band 6: £39,959 - £48,117
37.5 Hours Per Week
12 Month Secondment
Responsibilities
* Assess, treat and evaluate clients in a rehabilitation setting based on a patient‑centric programme.
* Prevent unnecessary future hospital admission and facilitate swift discharge to home.
* Work within a multi‑disciplinary team of nurses, occupational therapists, physiotherapists, assistant practitioners and rehabilitation assistants.
* Collaborate with community and acute wards, extended reablement and third‑sector services to avoid inappropriate long‑term care admissions.
* Provide consultations and education within primary and secondary care environments, offering formal and informal training to health‑care professionals and patients.
* Contribute to service development and policy alongside senior team members, and maintain clinical pathways.
* Supervise junior colleagues and students, providing specialist advice as required.
* Utilise the Calderdale Framework to promote skill sharing within the MDT.
* Engage in clinical supervision, CPD and training programmes.
Qualifications & Experience
Physiotherapist with experience in intermediate‑care rehabilitation, strong core physiotherapy skills and ability to work effectively within an MDT.
Additional Requirements
Must be able to fulfil the travel requirements of the post and may be subject to a DBS check.
