CHAW PCN Clinical Pharmacist

CHAWPCN has an exciting opportunity for an experienced Clinical Pharmacist to join our PCN, which covers practices across Chelford, Handforth, Alderley Edge, Wilmslow (CHAW) Primary Care Network.

We are looking for an experienced Clinical Pharmacist who ideally is a prescriber and has completed their CPPE course to join our current team of 6 pharmacists and 1 pharmacy technician. The role is full-time, working out of the PCN Hub located in Wilmslow Health Centre, with face-to-face consultations in each of the practices and very limited work from home.


Main duties of the job

The successful candidate will work across the 5 GP Practices in the PCN and will cover a range of pharmacy activities including clinical letters, patient queries, structured medication reviews, general medication reviews, and hypertension clinics.

Our aim is to provide exemplary patient care and find innovative solutions in general practice to deliver the best care for our patients. You will be supported in achieving this goal via our PCN Senior Clinical Pharmacist Lead and our PCN GP Trainer.

Job responsibilities

Job Purpose:

The post holder is a pharmacist who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. They will be supported by a senior clinical pharmacist who will develop, manage, and mentor them.

The post holder will work as part of a multidisciplinary team in a patient-facing role. They will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The post holder will provide primary support to general practice staff regarding prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice(s).

The postholder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies, so motivation and passion to deliver an excellent service within general practice are essential.


Key duties and responsibilities

Patient facing Long term condition clinics:

See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g., COPD, asthma).

Review the ongoing need for each medicine, monitor needs, and support patients with their medicines, ensuring optimal use (medicines optimisation).

Make appropriate recommendations to senior pharmacists or GPs for medicine improvement.

Patient facing Clinical Medication Review:

Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacists, nurses, and/or GPs on prescribing and monitoring.

Work with care home staff to improve safety in medicines ordering and administration.

Management of common/minor/self-limiting ailments:

Manage caseloads of patients with minor ailments within scope and limits of competence, signposting to community pharmacy and referring to GPs or other healthcare professionals as appropriate.

Patient facing medicines support:

Provide clinics for patients with questions, queries, and concerns about their medicines.

Telephone medicines support:

Provide a telephone helpline for patients regarding medicines questions.

Medicine information to practice staff and patients:

Answer medicine-related enquiries, suggest solutions, and follow up as needed.

Unplanned hospital admissions:

Review medicines associated with unplanned hospital admissions, conduct audits, and implement changes to reduce high-risk prescribing.

Management of medicines at discharge from hospital:

Reconcile medicines post-discharge, identify unexplained changes, and coordinate with patients and community pharmacists for continuity.

Signposting:

Ensure timely referrals to appropriate healthcare professionals for various needs.

Repeat prescribing:

Manage repeat prescribing processes, review requests, and ensure appropriate monitoring.

Information management:

Analyze medicines data, undertake audits, and implement safety alerts and guidance.

Implementation of guidelines and formulary:

Monitor prescribing against guidelines, assist in formulary maintenance, and promote adherence to NICE guidance.


Person Specification


Qualifications

* Minimum of 5 years post-qualification experience
* Minimum of 2 years primary care experience
* Mandatory registration with GPhC
* Masters in pharmacy (MPharm) or equivalent
* CPPE qualification
* Membership of Royal Pharmaceutical Society
* Post-graduate diploma or equivalent knowledge
