Overview
Band 5 Virtual Ward Staff Nurse – Sheppey Community Hospital, Sheerness. This opportunity is for a passionate and driven Band 5 Staff Nurse to join a dynamic healthcare team delivering high-quality, patient-centered care within the Virtual Ward setting, covering the SWALE area.
This role involves delivering and developing a service that supports patients to recover from or manage their illness and stay healthy for longer. You will work with the Senior Sister or Team Sister to assess and plan patient care, implement and evaluate treatment in collaboration with patients, and escalate changes in condition as needed.
Responsibilities
* Participate in assessment and care planning of patients on the virtual wards in conjunction with the Senior Sister/Team Sister.
* Implement and evaluate care and treatment in agreement with the patient and as defined in the care plan; escalate any deviation or change in condition to the Senior Sister/Team Sister/Charge Nurse/GP as appropriate.
* Communicate effectively with patients, carers and the multidisciplinary team.
* Prioritise daily work in line with clinical and patient need.
* Maintain accurate and legible patient records in line with professional and organisational requirements, including electronic patient record systems.
* Implement and work to ensure evidence-based care and best practice.
* Actively participate in team meetings and daily handovers.

Qualifications
* First Level Nurse Registration (NMC).
* Evidence of continuing professional development.
* Mentorship/teaching qualification (within first 2 years of post).
* Educated to degree level.
As this is a community-based role, a full UK driving licence and access to a vehicle for work purposes is essential.
