Join to apply for the AHP Support Worker role at NHS Tayside in Dundee Community Rehabilitation team. The Allied Health Professions work toward a 5 out of 7 day service while moving toward a 7 day service for improved patient outcomes.

Responsibilities
* Provide supportive role to Physiotherapy and Occupational Therapy colleagues within the Community Rehabilitation team.
* Manage a diverse caseload with clients who have a range of diagnoses, using an individualised rehabilitation approach to promote independence and quality of life.
* Link with statutory and third sector services to ensure a holistic approach to care.
* Deliver services across two sites in Dundee East and West Rehabilitation teams, contributing to a coordinated service for the people of Dundee.
* Support the development of knowledge and skills within the team and maximise the functional potential of clients.
* Work with referrals from hospitals within Dundee, GP practices, other health and social care providers, third sector providers and self-referrals from clients.
* Understand the roles of Occupational Therapy and Physiotherapy and demonstrate experience of working in a health or social care setting.
* Meet mandatory Induction standards for Healthcare Support Workers in Scotland and comply with the Code of Conduct throughout employment.
* Adhere to the planned 5 out of 7 day rota as NHS Tayside moves toward a 7 day service.
Qualifications
* Understanding of the role of Occupational Therapy and Physiotherapy and experience of working in a health or social care setting.
* Experience working in a health or social care setting.
* Ability to work within the team and contribute to a holistic approach to care.
* Willingness to achieve the mandatory Induction standards for Healthcare Support Workers in Scotland and to comply with the Code of Conduct.
