Job Description
We are seeking a dedicated and experienced Clinical Pharmacist to join a multidisciplinary healthcare team in Reading. This is an exciting opportunity to play a key role in optimizing patient care across various clinical settings. You will work closely with GPs, nurses, and other healthcare professionals to ensure safe, effective, and evidence-based use of medicines.

Key Responsibilities:

\n
* Provide expert clinical advice on medication management and prescribing practices
\n
* Conduct structured medication reviews, particularly for patients with long-term conditions or complex medication regimens
\n
* Support safe repeat prescribing systems and clinical audits
\n
* Help manage medicines-related queries from healthcare professionals and patients
\n
* Promote cost-effective prescribing and adherence to local/national guidelines
\n
* Contribute to the development of prescribing policies and clinical protocols
\n
\n
Requirements:
\n
\n
* Registered Pharmacist with the General Pharmaceutical Council (GPhC)
\n
* Minimum 2 years post-registration experience, ideally in a clinical or primary care setting
\n
* Independent Prescriber qualification (or working towards it) is highly desirable
\n
\n
Desirable:
\n
\n
* Experience working within a GP practice or primary care network (PCN)
\n
* Knowledge of EMIS Web or similar clinical systems
\n
* Commitment to continued professional development
\n
\n
