What the Job Involves As a Social Work Assistant based at County Hall, you will work under the guidance of the Team Manager to deliver essential services within the Adult Services Review Team. Your primary duties will include arranging and monitoring services in accordance with individual care plans, offering practical help and advice, liaising with various agencies, and conducting annual reviews of care plans. Participation in team meetings, training, and adherence to council policies is expected. While primarily based in the office, occasional visits to service users may be required to fulfill the role's demands.
