Care Coordinator – Anfield and Everton Primary Care Network
Role Summary: A Care Coordinator supports patients to navigate the health and care system, ensuring timely, personalised, and well‑coordinated care. The role focuses on reducing health inequalities, improving patient experience, and promoting preventative care, especially cancer screening uptake.
Key Responsibilities
* Perform personalised care planning and identify patient needs, goals, and barriers to engagement.
* Provide coordinated support for complex cases involving multiple long‑term conditions or challenging social circumstances.
* Facilitate navigation of GP, community, mental health, social care and voluntary services to ensure timely access.
* Deliver structured follow‑up, monitoring and escalation to maintain patient safety and continuity of care.
* Support patients to develop confidence, self‑management skills and a clear understanding of their care plans.
* Deliver targeted outreach to population groups with lower engagement or poorer health outcomes.
* Provide personalised conversations and reminders to increase uptake of national cancer screening programmes.
* Contribute to PCN initiatives aimed at reducing variation in access, experience and outcomes.
* Represent the PCN at community events, outreach activities and engagement sessions.
* Participate in multidisciplinary team (MDT) meetings and share insights for coordinated care planning.
* Maintain accurate, timely and comprehensive records of patient interactions, care plans and outcomes.
* Use clinical systems and digital tools to support care coordination, reporting and pathway monitoring.
* Ensure data is managed in accordance with confidentiality and information governance standards.
Qualifications
Essential
* Passion for improving patient care and outcomes.
* Strong interpersonal and communication skills, able to build rapport and trust.
* Ability to work with patients who have complex medical, psychological or social needs.
* Understanding of health inequalities and personalised care approaches.
* Ability to work independently and collaboratively within a multidisciplinary environment.
* Strong organisational and time‑management skills.
* Competence in using digital systems and maintaining accurate records.
* Confidence in community‑facing work and public engagement.
* Valid driving licence and own car.
Desirable
* Experience in a health, social care or community setting.
* Knowledge of PCN structures and ARRS roles.
* Experience supporting cancer screening or preventative health programmes.
* Experience in community engagement, outreach or population health work.
* Understanding of local voluntary and community sector services.
Remuneration and Working Conditions
* Salary: £27,942 per year.
* Contract: Permanent.
* Working pattern: Full‑time.
* Location: Priory Medical Centre, Belmont Grove, Liverpool, North West England.
