Job Summary
The Speech–Language Pathologist is responsible for implementing standards of care for speech language pathology services and ensuring adherence to all conditions in the Service Agreement.
Responsibilities
1. Conduct evaluations, assess the home environment (as appropriate), and identify equipment needs related to speech/language function.
2. Develop and update the individualized plan of care in collaboration with the patient, their representative, and caregivers.
3. Provide services ordered by the physician as indicated in the plan of care.
4. Offer counseling to patients, caregivers, and families.
5. Educate patients and caregivers about speech therapy and related care.
6. Prepare and maintain clinical notes.
7. Communicate with the physician responsible for the home health plan of care and other healthcare practitioners as needed.
8. Identify functional speech defects and establish plans to improve patient function.
9. Administer speech therapy programs using specialized techniques and equipment.
10. Recommend communication devices or aids as needed.
11. Participate in case conferences, in-services, and meetings to ensure coordinated care.
12. Maintain clinical competency in speech language pathology practice and theory.
13. Identify additional service needs for patients and refer appropriately.
14. Supervise home health aides in procedures related to the speech-language therapy plan of care, if applicable.
15. Contribute to quality assessment and performance improvement programs.
16. Perform other duties necessary to deliver speech language pathology services.
