Job Purpose
As a Promoting Independence Worker, you will play a vital role in supporting young people aged 16-19 as they transition into adulthood. Your work will empower individuals to achieve their personal goals and enhance their independence within the community.
Main Responsibilities
* Support and Empower: Assist young people in identifying and achieving their personal goals, promoting choice and control in their lives.
* Community Engagement: Facilitate access to social activities and community resources, helping individuals build connections and confidence.
* Skill Development: Teach essential life skills, such as using public transport and cooking, to enhance independence.
* Collaborative Care: Working alongside the PFA occupational therapist, with support from the Preparing for Adulthood Team and the Enablement
* Managers within the Maximising Independence Service (MIS), you will contribute to a collaborative working environment where the best outcomes for each person are at the heart of what we do.
* Strengths-Based Approach: Utilize a strengths-based methodology to understand and support the unique needs of each individual.
* Documentation and Reporting: Maintain accurate records of progress and outcomes, ensuring compliance with organisational standards.
* Training and Development: Participate in ongoing training and coaching to enhance your skills and knowledge in adult social care.
