Overview
We are seeking a dynamic and innovative individual to support the team to provide holistic and effective nursing care to people with learning disabilities requiring the support of Specialist Health Services, in order to improve health outcomes and reduce health inequalities for this service user group.
We have led the way on the development of frailty tools and the introduction of Physical Health Care and Treatment Reviews (PH-CTR) as part of our learning from LeDeR. The nursing team has been complimented on its approach and support to people with learning disabilities, including during the pandemic, and a team member has been nominated for an award for her part in this work. The STOMP clinic, led by a Non-Medical Prescriber and Queen's Nurse, is recognised by NHSE as good practice, and the post holder will be offered the opportunity to be part of this.

Responsibilities
* Work with people with learning disabilities who have additional physical or mental health needs and are unable to access mainstream health services.
* Support other services to make reasonable adjustments to improve accessibility and reduce health inequalities.
* Support desensitisation programmes to reduce health phobias and deliver education and training to other services and carers.
* Collaborate with partner agencies to deliver health promotion projects and to enable uptake of Physical Health Care and Treatment Reviews using a frailty tool.
* Coordinate annual health checks and contribute to Health Action Plans and Hospital Passports, working with GP practices and PCNs to improve uptake.
* Lead on LD Friendly GP practice accreditation with local PCNs support.

Job responsibilities
The Community Specialist Health Team is an MDT providing support in the community to adults with learning disabilities who require additional specialist support. The team supports access to mainstream services by developing pathways and providing education and training to other health services to enable reasonable adjustments and greater accessibility.
The post holder will work with the wider MDT to assess needs, develop care plans, evaluate outcomes, and promote empowerment through the process. They will provide high quality, individualised care addressing service user needs and collaborate with local partners to achieve service development outcomes in line with national policy and legislation.
The post holder will uphold the Code of Professional Conduct, provide holistic assessments and management of care packages, undertake risk assessments, maintain records, and ensure meaningful involvement of service users, carers and the MDT.
The post holder will advise on care for service users with challenging behaviour and mental health needs, lead groups when necessary, and ensure compliance with the Mental Capacity Act. They will maintain strong links with inpatient and community teams, GPs, and statutory/third sector partners to meet health needs close to home.
The post holder will coordinate and provide specialist education and training related to learning disability for staff across health, social care, residential care and third sector settings, and carry out clinical procedures as required.
The post holder will support with supervision, performance objectives, induction, and reflective practice, and promote positive experiences for people with learning disabilities when accessing mainstream healthcare services.
Person Specification
Education and Training
* Essential: Must have a relevant nursing degree
* Essential: Must have additional training in a specialist area
* Essential: Must demonstrate good understanding of relevant policy and reflect this in work
* Desirable: 2 years post-qualifying experience
* Desirable: Leadership skills and ability to support junior team members or students
Knowledge and experience
* Essential: Experience of working in the community
* Essential: Experience of working with people with learning disabilities
* Essential: Must be able to drive and have access to a car for work purposes
* Desirable: Awareness of LeDeR and its influence on service pathways
* Desirable: Ability to manage multiple complex tasks and work autonomously
