Overview

PHL ADHD Service is expanding. We are seeking experienced clinicians with Independent Prescribing qualifications to join our team. You will be responsible for conducting ADHD diagnostic assessments and managing treatment plans for patients aged 17.5 and over. This is a predominantly remote role with flexible working hours and opportunities to contribute to service development and clinical quality initiatives.


Responsibilities

* Independently conducting remote assessments for adult ADHD patients using referral information, screening tools, and clinical expertise
* Initiating and titrating ADHD medication in accordance with local and national guidelines
* Carrying out regular reviews to assess treatment outcomes and patient wellbeing
* Providing specialist clinical advice and empowering patients in managing their condition
* Supporting Shared Care arrangements with Primary Care providers
* Collaborating with internal and external professionals to ensure continuity of care
* Contributing to service improvement initiatives and participating in clinical governance

Job description and responsibilities

PHL provides a service to the Isle of Wight, receiving referrals for adults with symptoms suggestive of Attention Deficit Hyperactivity Disorder. Patients engage with triage, assessment and, if diagnosed, may proceed to pharmacological management of their condition. This is a virtual role for a non-medical prescriber with experience in assessing and treating adults with ADHD. You will work with the Clinical Manager and other professionals to diagnose and manage patients in a virtual clinic setting, including prescribing and titration of preparations in line with NICE guidance and clinical judgment.

You will have a sound understanding of the relevant NICE guidance (NG87) and quality standards (QS39) for ADHD in adults and experience using DIVA to diagnose ADHD. You must be confident in considering other psychiatric comorbidities when undertaking assessment and have a sound understanding of the therapeutic efficacy, side effects and interactions of prescribed treatments. You will work within a small team to provide excellent patient-centred care and participate in multidisciplinary meetings as required to review individual cases. This is a virtual role; assessments may be conducted by phone or video. PHL will provide full training and support to use the required applications and equipment.

Clinical

* Work autonomously within the wider team, demonstrating accountability, responsibility and professionalism when providing a quality service to our patients.
* Undertake assessments to determine whether a patient meets the criteria for an ADHD diagnosis, using clinical acumen and the DIVA tool.
* Formulate a management plan including treatment pathway, follow-up and review, discharge, and onward referral as necessary.
* Promote self-care and health strategies to empower the patient with a holistic, person-centred approach.
* Refer to the patient’s GP for vital signs or ECG monitoring in line with NICE guidelines.
* Refer to external agencies as needed (primary, community, non-profit and secondary care services).
* Identify safeguarding concerns and act in line with protocols.
* Record timely and accurate information in an electronic record.
* Perform risk assessments at each consultation with onward referral and risk management plans.
* Ensure timeliness of consultations within the booked appointment system and provide safety netting advice.
* Provide cover in the absence of the Clinical Manager as agreed and once confident and competent to do so.

Prescribing

* Prescribe appropriately for initial or repeat prescriptions in line with NICE guidance and the BNF.
* Conduct medication reviews, titrating dose or prescribing alternatives as needed.
* Work independently in reviewing medication and know when to refer for further management.
* Identify patients at risk of abuse or drug diversion.


Person specification

Qualifications

* Must be registered with the Nursing and Midwifery Council, HCPC or other appropriate professional body.
* Must hold an independent non-medical prescribing qualification and be confident with prescribing for off-label and controlled drugs.
* Maintains knowledge and training of current developments in clinical practice.
* Demonstrable experience of working with adults who have ADHD, including diagnosis and treatment.
* Experience of independent patient consultation, working autonomously.
* Sound clinical decision-making skills.
* Independent Prescribing qualification (V300)
* Minimum 2 years experience in ADHD diagnostic assessments
* Minimum 2 years experience of prescribing ADHD medication
* Comprehensive understanding of adult ADHD and related clinical guidelines
